How to register for Plan Management with Plan Tracker

2. Apply online
You can complete and submit your registration for Plan Management online by clicking the apply now button below.
3. Leave it with us
We’ll let you know when your registration is complete and any next steps you need to take from here onwards.
What to have ready
Have an NDIS number and plan showing the supports and services you are eligible for.
Have a copy of your NDIS plan with you so that you can find the information we need to set you up.
Have Plan Management included in your NDIS plan under Capacity Building Support – Choice and Control or Improved Life Choices. If you don’t already have it, you can request it from your Local Area Coordinator (LAC) or NDIA representative before you register.
Endorse Plan Tracker as your plan manager. For participants with new plans created after 30 October 2023 that have access to the new NDIS computer (PACE), please make sure that you contact the NDIA, your Local Area Coordinator (LAC), NDIS contact, or early childhood partner to endorse Plan Tracker as your plan manager.
You should be informed if you’re on the new computer system (PACE) during your plan reassessment and new participants will be notified that they’re on PACE during their planning meeting.

If you know you’re on the new computer system (PACE) please make sure you contact the NDIA on 1800 800 110, your Local Area Coordinator (LAC), or early childhood partner after completing your application to endorse Plan Partners as your Plan Manager and/or Support Coordinators.
