How to change to Plan Tracker in a few simple steps

Step 1:

Simply sign up with us and let your existing plan manager know you're changing providers. If you’re not sure what to say, feel free to copy and paste this text into an email for them:

Dear X

This is to advise I would like to cancel my service agreement with you for Plan Management. As per your service agreement, the notice period ends in [x] business days from today’s date.  

When closing the current service bookings in the NDIA portal, please reduce the available funds for all budgets to zero dollars to allow my new Plan Manager access to my funding.  

Please forward any unpaid invoices from my current NDIS Plan to [email protected] and confirm the date you will release my service bookings.

Step 2:

As soon as your existing provider cancels your current bookings in the NDIA portal, and the available funds for all budgets are set to zero, we can begin your service with us. Most providers have a 15-to-30-day notification period, but you can ask to have this reduced.

What happens after I am approved for the NDIS?

If you are eligible for the NDIS and have received an ‘access decision’ letter, the next step is creating your NDIS plan. You determine the goals you would like to set and the supports to help you achieve them. Your NDIA planner or local area coordinator (LAC) will help you through this process.

How do I make an NDIS plan?

You will create your plan with your NDIA planner or local area coordinator (LAC). To prepare, you will need to understand the services you want to access, the different methods to budget, who can help you start using your plan, how to find approved providers, and how to manage your plan.

What is an NDIS provider?
An NDIS service provider is a person, business or organisation who delivers funded services.
It’s important to work out what you want and find the right providers to meet your needs. Providers can include large companies, charities, small not-for-profits, sole traders, or any other type of business. Providers who are registered with the NDIS are called ‘NDIS registered providers’. They meet strict government quality and safety requirements.
If your NDIS funding is NDIA-managed, you can only use NDIS registered providers to deliver your services. If you have been working with a support worker or provider for several years and they are not an NDIS registered provider, encourage them to register so you can continue to work with them, using your NDIS funding.
Participants who self-manage or have a Plan Manager to manage their NDIS plan, can use NDIS registered providers and non-registered providers.
What is NDIS plan management?
Choosing to have your funding managed by an NDIS-registered plan manager such as Plan Tracker, means you have support for the admin and execution of your plan. Plan managers pay your invoices, help manage your budget, and offer guidance.
How do I include plan management in my plan?

Including plan management is simple, and is free to you. In the meeting with your NDIA planner or Local Area Coordinator (LAC) you can request to have your plan ‘plan managed’ and include ‘Improved Life Choices’ in your budget. If you are already on an active plan you can request a light touch review to add plan management.
