 A tool to support your decisions

When you’re making choices and decisions about how to spend your funding, it can be helpful to record your thoughts.

At Plan Tracker, we’re passionate about simplifying the NDIS. This FREE tool, with a list of questions to consider can help support your decisions about how you have chosen to spend your NDIS funding.

In October 2024, the NDIA introduced a new, improved process for reviewing claims and this tool may help provide evidence to support your claim.

How it works

1. Complete the My Supports Tool

The tool is a simple 7 step form that guides you through the questions from the NDIS Reasonable and Necessary Supports

By adding your NDIS number to the form, we will be able to save and link the information to your Plan Tracker account to refer to if needed.

Completing this may increase the chances of your claim being approved.

2. Submit your answers

When you have answered all the questions you’d like to record, simply click Submit the form.

You can use this tool as many times as you need to – in fact it’s a great idea whenever you need to make a decision.

Please note, if your enquiry relates to an ongoing expense you may need to answer these questions again.

3. Save for your records

Along with linking this form to your Plan Tracker profile, you will receive a copy on email for your records. You can save and share with whomever you need.

If you don’t see the email in your inbox within 10 minutes please check any social, promotional, junk or spam folders.

What you'll need to complete this form:

Your NDIS Plan including your NDIS number, Make sure you use your details registered with Plan Tracker

A copy of your quote or invoice.
R

Support Category –You can find these on your NDIS plan, Plan Tracker Portal or your ‘Week at a glance’ email.

Funding availability – Check you have enough funding available. Remember, your funding needs to last for the length of your plan. Up-to-date available funds can be found in your Plan Tracker Portal.

NDIS approved – Confirm that the support or service is covered by the NDIS. It should either be stated on your NDIS plan or NOT appear on the Supports Not Covered by the NDIS list.

Government supports – Check the support or service can not be funded through another government department (e.g. education, health etc).
